Consent for Minor to Travel

Use this letter when you are the parent or guardian of a child who is younger than 18 and are providing consent for that child to travel without you. Include pertinent details, such as the child's full name, age, date of birth, where he or she will be traveling to and from, dates of travel, whether he or she will be traveling with anyone else, and if so, who the travel companion will be. Most likely the child will carry a copy of this letter with him or her on the trip, and you should also keep a copy of this letter at your home in a safe place.
