Dear Ms./Mr. [Last name],

I am writing this letter in support of my application for a forbearance plan owing to my inability to pay the mortgage in full following an uninsured major medical emergency surgery (please see attached documents).

I have always considered myself blessed because I am fairly healthy and suffer none of the health issues that are common in many people of my age.
